FAQ - Patient

How do I obtain a copy of my medical record?
You will need to complete a HIPAA Authorization form and mail/fax directly to your physician’s office or HealthMark Group at (800) 833-5935. The HIPAA form can be downloaded from HealthMark Group’s website or you can complete this form in person at your physician’s office.
 
When should I expect my records to be sent?
HealthMark Group typically services your physician’s office once per week. In most cases your records will be mailed/faxed within 24-48 hours from the day we service your healthcare provider.
 
Is there a charge to obtain a copy of my medical records?
You may be subject to a processing fee when applicable. Medical record fees are in accordance with state law, a pre-payment invoice will promptly be sent to your attention. Once the invoice has been satisfied, your records are released to you or the designated recipient by mail, fax, or electronically.
 
Who can I contact with questions concerning the release of my medical records?
You may contact HealthMark Group’s customer service department at toll free (800) 659-4035. A Customer Support Specialist will be available to assist you, voicemails left are returned within 4 hours. You may also direct your questions and concerns through our contact page.
