Now you can pay and download your records online! Payment is accepted by Visa, MasterCard, or America Express. Once payment is processed you will have the option of downloading your records or having them mailed to your office.

INSTRUCTIONS

In order to pay online you must have the invoice from our office as well as the patient's date of birth. The date of birth is not published on the invoice and must be available to the requestor to verify identity.

Payment by credit card must be processed online. Credit card payments will not be accepted by phone or fax. Payment will be processed once you have verified the patient's name on the following page and specified how you would like to receive the records.

If your request requires original documents for certification you will only be provided the option to have the records mailed or shipped via FedEx. Outgoing shipments originating through this website will only be processed via FedEx. For other carriers you must provide your own documents and schedule pickup.
