HOW CAN I CHECK THE STATUS OF A MEDICAL RECORD REQUEST?
WHY CAN’T I DOWNLOAD RECORDS FROM MY MEDRELEASE LOGIN?
MedRelease is utilized to upload record requests and to access all information about all records leaving your medical facility. MedRelease360 is utilized for downloading records released from another medical facility. If you are a healthcare provider or staff member at a medical facility that has partnered with HealthMark, you’ll likely have access to both MedRelease AND MedRelease360. Depending on how you have established access to both, your login credentials could be the same, or they could be different. If you are a HealthMark client needing to download records requested from another healthcare provider, please login to MedRelease360 here.
HOW BEST SHOULD I CHANNEL ADDITIONAL QUESTIONS ABOUT A REQUEST?
HealthMark partners can engage directly and securely with HealthMark operations via the Questions tab within MedRelease. Questions submitted via MedRelease will receive a response in less than 10 minutes. You may also reach out directly to your Client Success Representative for additional support.
HOW MUCH DOES HEALTHMARK CHARGE FOR COPIES OF RECORDS?
HealthMark adheres to state statutes and federal rules when generating an invoice for records. These rates vary based on request type and location. Most patients will not receive an invoice for access to their own medical records. If you have any questions on if you will be charge for records, please contact 800-659-4035.
I CAN’T LOGIN TO MY MEDRELEASE ACCOUNT. WHAT SHOULD I DO?
HealthMark partners seeking to track status on requests for records leaving the hospital or medical practice can login to HealthMark’s MedRelease platform here. If you are having trouble logging in, try resetting your password here. If you still experience issues, reach out to your Client Success Representative.
HOW QUICKLY WILL I GET A RESPONSE TO A REQUEST FOR MEDICAL RECORDS?
HealthMark will respond to a request for medical records within 24-48 hours of receipt, if not sooner.
HOW DO YOU SUBMIT A REQUEST AND WHERE CAN A PATIENT GET A HIPAA AUTHORIZATION FORM?
Patients can submit a request for medical records or complete a HIPAA-compliant medical records release authorization via MedRelease360 by clicking “Submit Request”. Physicians, attorneys, and insurance companies can submit a request by faxing it to 800-833-5935 or by emailing the request to firstname.lastname@example.org. Please allow 24-48 business hours for processing.
IS IT POSSIBLE TO EXPEDITE A REQUEST FOR MEDICAL RECORDS?
HealthMark will respond to a request for medical records within 24-48 business hours. If you are needing a request expedited, please contact 800-659-4035 and speak directly with a customer service representative.
HOW CAN I PAY AN INVOICE?
Invoices may be paid online by credit card via MedRelease 360 here. You may also mail a check to HealthMark Group at P.O. Box 224688 Dallas, TX 75222-4688, or you can pay by phone by calling 800-659-4035.
WHY DO I HAVE TO SIGN AN AUTHORIZATION TO RELEASE A COPY OF MY MEDICAL RECORDS?
Federal regulations require a completed HIPAA authorization to fulfill any request for medical record in order to help protect patients’ personal and protected health information.
HOW WILL THE MEDICAL RECORDS BE RELEASED?
Medical records will be released according to the authorization or request. Delivery methods include secure digital download, fax, or mail. For digital downloads, a letter providing instructions on how to retrieve the medical records securely through our online portal will be delivered via email, fax, or mail.
WHAT ARE THE FEES ASSOCIATED WITH MY DISABILITY OR FMLA PAPERWORK?
The fee for processing disability or FMLA paperwork may vary based on the specifics of the request. An invoice will be delivered directly to the patient with further instructions. Should you have questions, please call 972-895-2138.
HOW DO I GET CHANGES MADE TO MY FORM?
If you need changes to your completed paperwork, please contact HealthMark Group at 972-895-2138 or email us at email@example.com. Upon approval from your healthcare provider, HealthMark will amend the necessary paperwork and reissue a new completed form.
HOW LONG WILL IT TAKE TO GET A COMPLETED FORM?
Patients should be contacted via email within 48 hours of submitting a request, if not sooner. Once HealthMark has all the necessary documentation from your healthcare provider, the requested paperwork will be processed. Final delivery of the completed form will require that both payment and a patient authorization are on file, at which time the completed form will be sent to the appropriate party.
HOW CAN I OBTAIN AN AUTHORIZATION FORM?
An authorization form can be obtained in your doctor’s office, or you can email firstname.lastname@example.org, and we will email you a form to complete. Yes, we take electronic signatures!
